The Halaxy Patient Forms Made Simple Workshop is a live, hands-on Zoom session designed for healthcare professionals, practice managers and virtual assistants who want to get their patient forms sorted once and for all. Whether you're starting from scratch, troubleshooting something that isn't working, or ready to build beyond your intake form, this is three hours dedicated to getting it done.
Created and hosted by Katrina, a sonographer and practice owner who works in Halaxy every single day, this workshop takes you through the process in real time. You'll plan what goes in your form, watch a live demo, then build it yourself with Katrina right there to help if you get stuck. If you need to share your screen to troubleshoot something confidential, you can jump into a private breakout room so no one else in the group can see your patient information.
Spots are capped at five people. That's intentional. Small groups mean you actually get help, not just access.

Part 1: Plan your form We start by working out exactly what your intake form needs to include and why. Katrina walks you through how to decide what to ask, what to leave out, and how to keep it clear for patients. You'll plan your own form as you go.
Part 2: Build it together Katrina demos how to create the intake form inside Halaxy, step by step. You follow along and build yours at the same time. If you get stuck, she's right there.
Part 3: Finish and troubleshoot Time to complete your form with support. If something isn't working the way you expect — sending, mapping, automating — this is the time to get it sorted. Need to share your screen privately? Jump into a breakout room and Katrina will work through it with you one on one.
By the end of the session, your intake form will be built, tested, and ready to use.
